SERVICES WE PROVIDE
Each student in the Potlatch School District must submit a Student Medical History Form. Please complete this form and return it to your student's school. The information provided to the school on this form allows us to develop individualized care plans for your student, should those be needed. It also keeps us informed of any health concerns you may have so we may better address the needs of our students. This information is kept confidential.

MEDICATIONS IN SCHOOL
A signed Medication Authorization Form is required for any medications to be given to your child at school. This form will also need to be signed by your medical provider for all prescription medications with dosage instructions. All medications, including over-the-counter meds, such as Tylenol or allergy medicine, need to be provided in the original container along with the Medical Authorization Form signed by the parent/ guardian with instructions for dosage, frequency and use.

Bell Schedule
School Hours: Students are permitted inside the school at 7:30 AM. Before this time, students must be under direct supervision of staff. Students need to make arrangements with staff to be in the building before 7:30 AM. The students must leave the building immediately following the instructional day unless they are with a staff member.

BUS INFORMATION
- For new students, please contact Sarah McKuin at the Bus Garage 208-875-0420 to determine your student's need for transportation services. Please include the student's name, school, and home address in your message.
SCHEDULE
- Elementary school buses arrive at the school at approximately 7:55 a.m. and leave the school at approximately 2:50 p.m.
- Middle/High school buses arrive at schools by approximately7:50 a.m.and leave the school at approximately 3:10 p.m.
ROUTES
- As part of our efforts to safeguard students, bus routes are not posted to the website. Please contact the Transportation Department at (208) 875-0420 for route information.

In the event it becomes necessary to close school due to extreme weather conditions, electrical failure, etc., students and parents are advised to listen to local radio and television stations. Information will also be posted on our website and parent information on file will be used to send automated phone calls (please make sure we have up-to-date contact information on file for you).
The decision to close will generally be made by 7:00 a.m
